The airfield dates from at least the mid-1970s in its civil role, being put into operation for combined civil and military aviation in March 1976, and its landing strip was later extended in 1994 to take medium-range aircraft. Phú Bài officially welcomed its first international flight on 30 October 2005, a charter operated by Austrian Airlines bringing tourists from Luang Prabang in Laos, following government approval for international flights in 2002. In May 2007 Changi Airports International signed a memorandum of understanding with the Thua Thien Hue provincial authorities to develop the airport as part of a wider tourism master plan for the province, and the field formally became Vietnam's fourth international airport that August. The runway underwent a major repair and upgrade programme in 2013, closing from 20 March to 20 September of that year at an estimated cost of 500 to 600 billion Vietnamese dong; during the closure, air travellers to Huế used Da Nang International Airport instead. A new passenger terminal, funded by the Airports Corporation of Vietnam at a cost of around US$96.9 million, was subsequently built to expand capacity toward five million passengers a year by 2020 and seven million by 2025.
Phu Bai continues to operate as a joint civil-military airport handling scheduled domestic services, with airlines including Vietnam Airlines, VietJet Air, Pacific Airlines and Hai Au Aviation using the field; there are currently no direct international routes, with connections to international destinations requiring a transit through Hanoi or Ho Chi Minh City.
